Alsager Netball Club is committed to providing a safe, positive and supportive environment for everyone involved in our club.
This form can be completed by any player, parent or carer, coach, volunteer, official or other person who has a safeguarding or welfare concern. You do not need to be certain that something is wrong to raise a concern. If something you have seen, heard or been told has caused you concern, please report it.
Please provide as much factual information as possible. Do not investigate the matter yourself or ask leading questions.
All concerns will be treated seriously, sensitively and shared only with those who need the information to safeguard the individual concerned.
If someone is in immediate danger or requires urgent medical attention, contact the emergency services on 999.
